This study investigates the molecular and phenotypic mechanisms behind midostaurin resistance in FLT3-mutated acute myeloid leukemia (AML). Researchers identified a specific progenitor-like cell population marked by CD45RA and CD38 expression that facilitates survival through a signaling shift from STAT5 to PI3K/AKT. Using multi-omic profiling and functional drug testing, the authors discovered that these resistant cells exhibit disrupted membrane architecture and distinct apoptotic thresholds. The findings suggest that combining midostaurin with SMAC mimetics can restore cell death and selectively eliminate this resistant subpopulation. Furthermore, the study indicates that leukemic differentiation states are stronger predictors of drug response than genetic mutation status alone. This research highlights functional precision medicine as a vital tool for identifying novel therapeutic combinations to overcome treatment failure in AML.
