The reviewed literature introduces the Stiffness-responsive MSC-Assisted Relayed tumor-Targeting (SMART) platform, an innovative therapeutic and diagnostic approach designed to overcome the lack of molecular targets in challenging cancers. By engineering mesenchymal stem cells (MSCs) with a mechanosensitive circuit driven by YAP/TAZ, the system successfully translates the physical stiffness of the tumor microenvironment into the expression of actionable synthetic biomarkers like PSMA. This programmable cellular strategy effectively "tags" previously untargetable tumors in situ, enabling sensitive PET imaging and targeted radionuclide therapy across diverse cancer types and metastatic models. Furthermore, this technique demonstrates superior diagnostic sensitivity compared to conventional methods such as 18F-FDG PET, successfully expanding the scope of precision radiotheranostics beyond native biological markers.
